5 Way Rotary Switch Question

hi i want to hook an hsh pickup confifuration that was orginally hooked up to a 5 way blade switch to a 5 way rotary swith. here is the orginal diagram with the blade switch. How do I hook that up to a 5 way rotary switch? Also do i want a 2 pole or 4 pole rotary switch? I have never wired a rotaRY SWITCH BEFORE SO I HAVE NO IDEA WHAT THE VARIOUS LUGS ARE FOR.

you need just 1 pole, so get the 2 pole.

i can only find a 5way 4 pole switch would the wiring be the same?

Additionaly what would the switch positions be , what pickups would be active at each position?

60 would be ideal if some one could draw that into something i can read, like rubber314 did.

yeah, my bad... I wasnt thinking when I drew it... Sock Puppet has it right.

all the connections are on the bottom lugs correct with the 4 pole 5 way switch?
