Why not put up a photo, that ought to help. As for the template, it's not all the difficult to come up with one on your own, just make sure it fits whatever cover you're planning to use. Chances are that (assuming you have a real Gibson body) it is a Les Paul Supreme. The wiring harness in those guitars is fed through a big hole underneath the oversized output jack plate, it creates a pretty cool look having a continuous back plate with no control cavity.
