In that case, the numbers refer to the frets you play on each string. When the all the numbers are in a vertical line, they're played as a chord. The slide should be obvious (though it's often written just as "s" or "sl") I'm guessing the 'x' before the 12 is a muted note/sound/tap, you're not supposed to let the note ring out, just the percussive sound. Tab rarely gives rhythmic information, you're supposed to listen to the song and figure that out yourself. Keep in mind that most tab on the net is completely wrong. In my experience, anyway.
